Please be present one hour before your scheduled time.
Test application deadline is 14 days before scheduled test date.
No refunds once the deadline has passed.
Returned checks will result in a $30.00 fee.
There will be a $30.00 late fee for any applications received after the deadline.
Parents are not allowed in the rink during the test, but must wait in the lobby.
Only coaches who are currently registered with USFSA will be allowed in the rink during test.
